Online Entry Form Instructions:

These instructions are for both individual and Group entry forms

1. Select Individual or Team/Corps Entry Form button from above. 

2. VERY IMPORTANT STEP to protect your privacy: 

   • Before you fill out your entry form, select File from the top left, scroll down and select MAKE A COPY.

   • Rename that copy with the contestant's name or group name.

   • Then use that renamed copy to enter your data. This will protect your personal information.

3. Once you've filled out the renamed copy, select File from top left.

4. Scroll down to Email and Select email this file.

5. Email entry form to kentuckytwirlingconsortium@gmail.com. You can either email it directly from Google Sheets or resave, open your email app, attach entry form and send. Remember to download and/or print a copy for yourself.

6. Once you have emailed entry, payment must be made via one of the following methods:

   • • Venmo - @Tami-Moore-42  

     • Facebook Pay – Tami Hankins Moore

     • Mail  ($50 charge for NSF checks)

7. Remember to pay as family and friends to avoid being charged a fee for using the app. 

8. If you do not have one of the 2 payment apps please send a check payable to Tami Moore post marked by April 1st.

Paper/Mail Entry Form Instructions:

These instructions are for both individual and Group entry forms

1. Select Individual or Team/Corps Entry Form button from above. 

2. Click on Paper form tab at bottom of spreadsheet.

3. VERY IMPORTANT STEP to protect your privacy: 

   • Before you fill out your entry form, select File from the top left, scroll down and select MAKE A COPY.

   • Rename that copy with the contestant's name or group name.

   • Then print that renamed copy. If both the online version and paper form download when you make a copy, simply print the pages that contain the paper version.

4. Fill out the paper copy. Then either drop it in the mail to

5.  Scan, save & email entry form to kentuckytwirlingconsortium@gmail.com . Remember to download and/or print a copy for yourself.
